This alarming case of infectious endophthalmitis is just 4 days after cataract surgery. The eye is red, swollen, and painful, and vision is greatly decreased. Eyelids are inflamed, swollen and painful to touch. The conjunctiva is inflamed, swollen and hemorrhagic from the recent cataract surgery. A large mucous strand made white with white blood cells drapes from an unseen limbal suture across the lower cornea toward the nose. The cornea and anterior chamber are largely opaque from inflammatory white blood cells and corneal stromal edema. The iris and pupil are barely visible. Normal red fundus reflex is gone.
