58 year old male patient with symptoms of blur and glare, especially when driving, worsening over the last year. Vision was beginning to affect daily quality of life and he was interested in discussing cataract surgery. Patient was very interested in decreasing his dependency on optical corrections. History of mild amblyopia OS
Objective
Pre-op Rx:
     OD +8.75-3.24x162, 20/25+
glare acuity 20/80
potential acuity 20/20
     OS +8.25-3.00x025, 20/40+
glare acuity 20/400
potential acuity 20/30
SLE:
     unremarkable except for 2+ nuclear
sclerosis and 1+ cortical opacity OD,
1+ nuclear sclerosis and 2+ cortical opacity OS
IOP:
     17/18
Dilated Fundus Exam:
     C/D 0.5, all else unremarkable
Assessment
Cataracts, OS>OD, causing symptoms affecting daily activities, OS mild amblyopia
Plan
Risks/benefits of cataract surgery reviewed and patient chose to proceed with CE with IOL 8-8-00 OD, 8-16-00 OS. Interested in LASIK to correct residual astigmatism.
